An opportunity is available for an Infusion Community Health Nurse.

Essential Job Functions:
  1. Performs the assessment function for all clients and derives nursing diagnoses based on assessment data. Provides care focusing on patients receiving infusion therapy. Initiates, monitors, cares for and tracks all IV lines, VAD’s and PICC lines and therapies, including but not limited to, antibiotics, immune therapy, chemotherapy via IV mode.
  2. Plans nursing interventions in collaboration with the interdisciplinary team to resolve current problems and prevent potential problems. Involves the patient/family in determining care goals and objectives. Acts as a patient advocate.
  3. Demonstrates and instructs care to be provided by the patient, family, or caregiver and provides ongoing supervision and instructing as warranted.
  4. Identifies need for intervention by other members of the interdisciplinary team on an ongoing basis. Reports changes in patient condition to the attending physician and seeks needed changes in pharmacological intervention or plan of care, as warranted.
  5. Provides on-site orientation and supervision of HHAs on an ongoing basis. Documents and reports to appropriate supervisor any identified performance issues.
  6. Identifies and articulates essential components of home health benefits as administered under Medicare and other payors and demonstrates an understanding of home health reimbursement methodologies to manage a caseload and deliver care in a cost-effective manner.
  7. Participates in the development of policies, procedures, clinical protocols, forms, and service programs as requested. Participates in departmental and/or agency-wide quality improvement activities.
  8. Seeks opportunities for professional growth, including demonstrating the ability to receive constructive performance feedback.
