An opportunity is available for a Speech and Language Pathologist.

Essential Job Functions:
  1. Evaluates patient’s speech articulation and fluency, voice, swallowing, language processing, non-oral communication, higher level language functioning, safety risks (including airway protection), device and equipment needs, available support systems and rehabilitation potential. Completes Start of Care visits when directed by manager.
  2. Develops, implements and evaluates speech and language pathology plan of care with patient and caregiver involvement and implements it as approved by the physician. Develops strategies to improve swallowing, speech, language and communicate skills. Initiates changes in the plan of care as appropriate striving for agency results.
  3. Teaches patient, caregivers, and other professional visiting staff, as appropriate, therapeutic techniques to ensure the patient’s safe and highest level of function consistent with the approved plan of care. Recommends and makes referrals to community resources and other disciplines as needed to achieve agency results for patient care.
  4. Recommends necessary adaptive and safety equipment and supplies for patient to purchase. Contacts physician as needed for orders. Orders and teaches use of equipment and supplies.
  5. Monitors vital signs, pain and significant changes in the patient’s physical and mental status. Reports changes to physician, case manager and therapy manager. Implements change in plan of care when ordered by the physician.
  6. Performs case management duties when a nurse or physical therapist is not in a case according to agency processes. Coordinates care with other team members by communicating both verbally and in writing about discharge planning, recertification, transfer to hospital and other issues as required by agency processes. Participates in case conferences.
  7. Prepares daily activity reports, baseline and other clinical documents, visit notes, progress summaries and discharge summaries in a complete, accurate and timely manner as required by third party payors and agency processes focusing on agency results.
  8. Manages a caseload effectively and delivers care in a cost-effective manner and in compliance with third party payor requirements.
